Job Summary
Coordinate with health system providers and community-based resources to improve maternal health outcomes for University Health patients. Work with clinical care teams and social service agencies to assist patients through prenatal, delivery, and postpartum services. Provide screening, education, support, and care coordination, ensuring appropriate information is disseminated and patients are navigated to medical appointments with necessary follow-up. Manage patient tracking, data entry, and reporting across clinical and community settings. Serve as a liaison within the Baby Bexar Healthy Families Community-Based Perinatal Health Initiative.
Required Qualifications
- Bachelor's degree in social sciences, health education or related field of study
Desired Qualifications
- Four years of experience in the area of health education, public health, social work and/or case management
- Community Health Worker certification
- Knowledge and experience working with programs of similar scope and focus
- Spanish fluency
