Job Summary
Conduct ongoing care coordination and case management for patients with substance use disorders. Guide linkage to clinical services including medication-assisted treatment, behavioral health, and social services while providing education and support. Meet patients in emergency departments, addiction treatment appointments, and community settings to foster trust and encourage treatment retention. Identify health and wellness needs, barriers, and strengths across physical, social, and financial domains. Perform outreach as an advocate for patient services and attend required clinical and programmatic conferences.
Required Qualifications
- High School Diploma REQUIRED
- Registered Nurse
- Licensed Practical Nurse with 2 years lived experience
- Baccalaureate (BA) degree with 2 years lived experience
- Associate degree with 4 years of lived experience
- Certified Medical Assistant with 4 years of lived experience
- 1 year of relevant experience if candidate has bachelor's degree in related field
- 2 years of relevant experience if candidate has associate's degree or is a Certified Peer Recovery Specialist
- 4 years of relevant experience, either clinical or lived, if candidate is a Certified Medical Assistant
- 4 years of relevant experience, either clinical or lived
- Ability to communicate with patients, visitors and coworkers
Desired Qualifications
- Lived experience with navigating the criminal-legal system preferred
- Bi-Lingual preferred
- Bilingual candidates preferred
