Navigator RN - Case Management

RN Care Navigator manages patient populations with chronic illness and multiple co-morbidities; coordinates with multiple PCPs and serves as the central link to an identified population of patients. Through skilled clinical assessment, patient education, collaboration and coordination of healthcare and community resources, the RN Care Navigator assists patients to gain self-efficacy/management skills, achieve optimum functional health status, and quality of life. This role works under minimal supervision within standard policies and the nursing code of ethics and may perform other ambulatory care RN duties as required.
