Job Summary
Provide disease-specific education, referral navigation, and ongoing support to patients and families while expediting access to diagnostic and treatment procedures. Connect individuals with appropriate resources and address issues of care access by collaborating with social work, financial counselors, chaplains, dietitians, and the heart team. Share knowledge regarding treatment options and respond to patient needs as part of the navigation team at Providence Medical Group Montana.
Required Qualifications
- Graduate of an accredited school of nursing
- Associate's Degree Nursing
- Montana Registered Nurse License
- National Provider BLS - American Heart Association
- 3 years Professional nursing experience
- 1 year Nurse navigation or similar experience
Desired Qualifications
- Bachelor's Degree Nursing
- Related experience in an ambulatory setting, preferably in a medical office
