Job Summary
The Navigator I will enhance clients' likelihood of success by identifying and reducing barriers, conducting outreach and engagement, performing case management, and coordinating wrap-around supportive services. Responsibilities include documenting and tracking patient interactions per AAC, ADHS/DBHS Provider Manual, CARF, and organizational policies; collaborating within a multi-disciplinary team; mapping next steps for individuals to access services and achieve ongoing success. The role emphasizes behavioral health and crisis-related support, with duties spanning outreach, assessment, and linkage to residential, outpatient, and other services.
Required Qualifications
- High school diploma or GED
- Associates Degree (or higher) in a field related to Behavioral Health preferred
- Driver's License (valid and good standing)
- Clear 39-month Motor Vehicle Record
- 1-3 years of health care related work experience preferred
- 1-3 years of substance abuse/mental health recovery-oriented service preferred
- Minimum six months of recovery from substance use and/or mental health disorders preferred
