Navigator II - Day Shift

Job Summary
Conduct outreach, engagement, and case management for individuals requiring additional support to identify barriers and connect them with needed services. Map appropriate next steps for clients accessing various resources while documenting and tracking patient interactions per Arizona Administrative Code, ADHS/DBHS Provider Manual, CARF, agency policies, and program specifications. Collaborate with community members, local providers, and internal stakeholders to distribute resources and share information during peak heat periods from May to October. This seasonal role supports vulnerable populations affected by extreme heat within a multi-disciplinary team of internal and external stakeholders.
Required Qualifications
- Highschool diploma or GED
- Minimum of 12 months of recovery from substance use and/or mental health disorders
- Minimum of ninety (90) days experience in a Navigator or Peer Support Specialist role
Desired Qualifications
- Associate degree (or higher) in a field related to Behavioral Health
- 1-3 years of full-time Health Care related work experience where his or her prior experience would provide adequate exposure to both behavioral and medical crisis situations
- 1-3 years of substance abuse/mental health recovery-oriented service
