Job Summary
Conduct outreach, engagement, and case management to connect clients with needed services and resources. Map out appropriate next steps for individuals accessing various services to achieve ongoing success. Document and track required patient interactions according to Arizona Administrative Code, Department of Behavioral Health Services Provider Manual, CARF, and agency policies. Collaborate with community members, local service providers, and internal teams to distribute resources and share information during peak heat periods from May to October.
Required Qualifications
- Highschool diploma or GED
- Must be in recovery from Alcohol or Drugs and/or mental health for equal to or greater than 1 consecutive year
- Current AZ Driver's License valid and good standing
- Clear 39-month Motor Vehicle Record
- Arizona Fingerprint Clearance card or good cause exemption
Desired Qualifications
- Associates Degree (or higher) in a field related to Behavioral Health
- 1-3 years of full-time Health Care related work experience where his or her prior experience would provide adequate exposure to both behavioral and medical crisis situations
- 1-3 years of substance abuse/mental health recovery-oriented service
