Job Summary
Conduct outreach, engagement, and case management for individuals requiring additional support, mapping appropriate next steps to access services and reduce barriers. Document and track required patient interactions according to Arizona Administrative Code, ADHS/DBHS Provider Manual, CARF, and agency policies. Collaborate with community members, local providers, and internal teams to distribute resources and connect people in need with appropriate services during peak heat periods from May to October. Work within a multi-disciplinary team of internal and external stakeholders to enhance client success likelihood.
Required Qualifications
- Highschool diploma or GED
- Must be in recovery from Alcohol or Drugs and/or mental health for equal to or greater than 1 consecutive year
- Current AZ Driver's License valid and good standing
- Clear 39-month Motor Vehicle Record
- Arizona Fingerprint Clearance card or good cause exemption
Desired Qualifications
- Associates Degree (or higher) in a field related to Behavioral Health
- 1-3 years of full-time Health Care related work experience where his or her prior experience would provide adequate exposure to both behavioral and medical crisis situations
- 1-3 years of substance abuse/mental health recovery-oriented service
