Job Summary
Navigator, Full-Time service coordination and navigation for youth aged 14-24 in Woodlawn, Washington Park, and Grand Boulevard. Provides trauma-informed case management, intake and assessment, service planning and goals development across housing, education, employment, and well-being. Builds youth networks, coordinates referrals and services, conducts street outreach and community engagement, schedules check-ins, documents cases, creates transition plans, and may supervise volunteers/interns. Requires valid driver's license, clean driving record, willingness to drive agency vehicle, and successful background check. Reports to SCaN Program Manager and supports program planning, training, and team collaboration.
Required Qualifications

- Willingness and ability to drive agency vehicle when requested
- Background check required and must be insurable under agency coverage
