Job Summary
Document patient interactions in accordance with ROHCG health care records practices and policies. Assist in daily home/unit activities, including patient counts, meal supervision, refreshments, and other activities as directed by the interdisciplinary team. Monitor inventory, coordinate supply reordering, and communicate regarding supplies and equipment. Support environmental safety, equipment maintenance inspections, infection control practices, and proper use of personal protective equipment. Transport patients, supplies, equipment, mail, requisitions, specimens, and other items as required. Maintain the tidiness, cleanliness, and safety of patient care areas, including assisting with patient rooms and ensuring areas remain free of hazards. Supervise and monitor patient movement within the home/unit and in the community as required; report all patient movement and observations to the interdisciplinary team. Support patients with daily living activities, meal supervision, social and leisure activities, and other assigned tasks. Assess and report patient interactions and share responsibility with the team for patient care and maintaining a safe environment. Participate in program evaluations, discharge planning, implementation of discharge plans, and assist residents with discharge-related applications. Communicate with community agencies and families as directed by the interdisciplinary team. Maintain patient confidentiality and contribute effectively as a member of a multidisciplinary team. Adhere to ROHCG policies and procedures, staff and patient safety requirements, and The Royal's Anti-Racism, Harassment & Discrimination Free Policy.
Required Qualifications
- Minimum 1 year post-secondary diploma or certification in health related field
- Knowledge of infection control principles and use of personal protective equipment
- Ability to participate as a member of a multidisciplinary clinical team and function well in a team oriented climate
- Perform clinical work functions that support the provision of patient and family focused care
- Certified in CPR and First Aid
- Satisfactory Criminal Reference Check – vulnerable sector
- Possess a valid G driver's license
- Demonstrated ability to work effectively both independently and as a member of a professional multi-disciplinary treatment team
- Safety and Security in the Workplace Training
- English level A- is mandatory in oral expression, oral comprehension, reading comprehension and written expression
