Multi-Service Navigator

Manage a caseload of children in foster care, ensuring their safety and well-being by developing individualized treatment plans focused on permanency and stability. Support foster parents with parenting strategies, maintain consistent communication with families and service providers, and represent cases in Family Court. Document progress and maintain compliance with agency and ACS standards while participating in intake activities to assist treatment teams with referral recommendations. Facilitate internal referrals, attend regular treatment meetings to identify resources, and monitor cross-divisional referrals via the Electronic Health System. Identify and develop partnerships with community-based service providers, train staff on the full continuum of CMCS services, and support data collection in the multiservice dashboard. Bilingual in Spanish preferred.
