Job Summary

Conduct initial interviews with clients and families to assess eligibility for Medicaid Alternative Care, Tailored Supports for Older Adults, and Family Caregiver Support Program services. Assemble group discussions with professionals and program representatives to formulate goals and design integrated intervention plans. Monitor adherence to plans, manage information flow within the action system, and provide follow-along support to identify unexpected service delivery problems. Maintain coordination momentum and goal orientation while performing related duties as assigned.

Required Qualifications

  • Four years of paid, full time, on-the-job social service experience in providing care management and long-term support and services to vulnerable adults, such as providing one-on-one, in-person customer service, developing care plans for aged, blind, or disabled individuals and caseloads
  • High school diploma or its equivalent
  • Three years of paid, full time, on-the-job social service experience, providing one-on-one, in-person customer service, developing care plans for aged, blind, or disabled individuals and caseloads, a high school diploma or its equivalent, and are bilingual or bicultural with such experience
  • Two years of experience with other DSHS information systems used by Case Management staff, such as Comprehensive Assessment and Reporting Evaluation (CARE) tool, Barcode, ProviderOne, QA Monitor, ACES.Online, or ALTSA reporting
  • College courses, an Associate's degree, or its equivalent
  • Two years of paid, full time, on-the-job social service experience and Bachelor's degree
  • One year of paid, full-time, on-the-job social service experience and a Bachelor's degree in social services, behavioral or health sciences
  • Social service experience and a Master's degree
  • One year full-time equivalent verifiable hours through an internship or volunteer work in social services
  • Valid driver's license in state of residence
  • Auto insurance in the amount required by the State of Washington
  • Acceptable completion of a criminal history background check
  • Completion of state-sponsored CORE training
