MSW / Social Worker - Hospice

Job Summary
Assess psychosocial needs of patients and families, evaluate home safety, and assist with alternative care plans when needed. Provide counseling, crisis intervention, and emotional support while maintaining accurate clinical documentation. Educate families on advance directives and provide referrals for community resources. Collaborate with team members to support holistic patient care and assist with funeral, financial, and legal planning. Serve as a liaison with community agencies to ensure psychosocial needs are met with dignity. Work M-F 8-5 in the Deland & Surrounding Area. Requires MSW or BA in social work with current licensure and one year of healthcare experience.
Required Qualifications
- A graduate of a master's program in social work (MSW) accredited by the Council on Social Work Education
- A graduate of a bachelor's program in social work accredited by the Council on Social Work Education
- Meets personnel qualifications stated in §418.114(b)(3)
- Minimum of one (1) years' experience in health care
- Current licensure according to Medicare guidelines to practice in state of operation
- Must be a licensed driver
- Must have an automobile that is insured in accordance with state or organization requirements
- Must have an automobile that is in good working order
