Job Summary
Hospice Social Worker providing psychosocial assessments and counseling for patients with terminal illness and their families; conducts social evaluations, educates on advance directives, and coordinates information and referrals with community agencies. Responsibilities include crisis intervention, liaison with the interdisciplinary team, maintaining patient records, planning for funeral and related responsibilities, and participating in quality improvement activities. Requires flexibility in work hours and travel throughout the Bristol Hospice service area; individuals should understand hospice philosophy and be capable of supporting patients and families through end-of-life concerns. Travel-ready and able to collaborate with a broad network of professionals to support patient care.
Required Qualifications
- Master’s degree in social work from an accredited university
- Minimum of two (2) years of documented supervised experience in health care; hospice experience preferred
- Must understand hospice philosophy and issues of death/dying
- Certified Hospice and Palliative Social Worker desirable
