MSW/LCSW PACE

MSW with LCSW licensure sought to join NeighborHealth’s Neighborhood PACE program as a PACE Social Worker. The role involves providing clinical social work and case management for Older Adults enrolled in PACE, conducting psychosocial assessments, developing and updating individualized care plans, coordinating interventions with the Interdisciplinary Team (IDT), facilitating caregiver support and family meetings, leading clinically informed activities (group counseling, end-of-life support, resource coordination, intern supervision, ethics/complex case consultation), and supporting hospital/SNF admissions and care transitions. The position emphasizes advocacy, documentation, and collaboration with participants to align goals of care, with frequent local travel between East Boston and Winthrop sites.
