Job Summary

Social Worker (MSW) at Kaiser Permanente NW providing psychosocial assessments and supportive services to members. Responsibilities include conducting age-appropriate assessments, developing cost-effective health care plans utilizing member resources and community programs, promoting patient-centered, multidisciplinary care, maintaining knowledge of KP services and relevant laws, documenting services and care plans, and contributing to program development and education to empower members in directing their health care. Requires MSW degree, state licensure credentials, and BLS; experience in hospital/outpatient/long-term care settings and ability to collaborate with multidisciplinary teams.

Required Qualifications

  • Masters degree in social work from an accredited university
  • Credentials from multiple states as required at hire leading to Oregon/Washington licenses
  • Basic Life Support (AHA) within 3 months of hire
  • National Provider Identifier required at hire
  • Experience in hospital/medical outpatient clinic/home health/hospice/long-term care setting
  • Minimum one year of Social Work experience in relevant settings OR completion of Kaiser Permanente NW Social Worker Internship program
  • Minimum three years of additional related experience
  • Licenses/certifications: LCSW or MSW license or equivalents per state requirements
  • Ability to work effectively on a multi-disciplinary team
