Job Summary
Conduct comprehensive evaluations of patients' social, emotional, cultural, and financial circumstances in collaboration with the healthcare team to recommend intervention plans aligned with patient goals. Coordinate patient care appropriate to the age of individuals served on the assigned clinical area while assisting families with personal and environmental difficulties related to predisposed illnesses or barriers to medical care. Provide social services under the supervision of a licensed social worker, clinical social worker, or registered nurse in accordance with hospital policies. Complete job-specific orientation and meet performance appraisal standards for Social Services competencies.
