Job Summary
Provide HIV Navigation Services for People Living with HIV/AIDS and individuals engaging in high-risk behaviors by conducting assessments, developing client-centered action plans, and inputting data into AIRS and Cap60 systems. Link and retain clients in medical care, treatment, and appropriate resources while referring them to HIV/STI/HCV testing, screening, and PrEP/PeP services. Maintain client records, progress notes, and monthly AIRS reports in accordance with agency policy, and participate in quality improvement activities. Collaborate with the Targeted Prevention Educator to promote comprehensive program delivery and attend community events to increase awareness of testing and screening resources.
Required Qualifications
- Extensive knowledge of HIV/STI infection, transmission and prevention
- Knowledge of community resources and referrals for HIV/STI's, mental/behavioral health, care coordination, and appropriate client-centric referrals
- Ability to work as part of a team with co-workers and with other colleagues as necessary
- Minimum two years' experience in HIV/STI prevention or related field
- Ability to follow directions (orally and in writing)
- Bachelor's degree in Community Health Education or related field
- Two years of experience
- 3 to 4 years related field experience
- Must have extensive knowledge of HIV/AIDS/STI's transmission and prevention
- Must be knowledgeable about HIV/AIDS prevention in diverse communities including MSM of color, women of color, injection drug users, and persons who exchange sex for resources
- Willing to work evenings and weekends ( flex-time provided )
