Job Summary
RN Case Manager for Mother/Baby & Pediatric integrated care leads and coordinates inpatient to post-hospital care, collaborates with the multidisciplinary team, ensures timely and cost-effective care, facilitates discharge planning and resource referrals, advocates for patients and families, maintains licensure and engages in performance improvement activities, participates in daily ICC huddles, and balances clinical and financial outcomes while aligning with payer requirements.
Required Qualifications
- Required Education and Experience: High School Graduate; Bachelor Degree in Nursing; 2 or more years clinical experience in a health care setting
- Licensed Registered Nurse (LRN) - Single State - State Board of Nursing; Registered Nurse in State of Kansas
- Basic Cardiac Life Support (BLS or BCLS) - American Heart Association (AHA)
