Job Summary
Develop and maintain strong relationships with individuals receiving support, their families, home providers, and direct care workers. Help individuals express their goals and needs while ensuring their voices are heard. Monitor relationships between individuals and home providers to ensure positive matches. Collaborate with the service planning team to create Individual Support Plans reflecting unique wishes and needs. Oversee service delivery and contractual compliance by completing visits in the home or service environment. Provide on-call support to Mosaic at Home Providers and Direct Care Workers as needed. A Bachelor's degree is preferred along with two years of related work experience. Valid driver's license required.
Required Qualifications
- A minimum of two years of related work experience
- Valid Driver's license
- Experience in behavior management
- Knowledge of regulations
- Strong communication and relationship-building skills
- Excellent planning and problem-solving skills
- Computer proficiency
