Develop and maintain relationships with individuals receiving support, their families, home providers, and direct care workers to ensure their voices are heard and preferences respected. Collaborate with the service planning team to create Individual Support Plans reflecting unique wishes and needs while overseeing contractual compliance. Monitor matches between individuals and providers, complete visits in home or service environments, and provide on-call assistance to resolve problems as needed. Requires a bachelor's degree, two years of related experience, and a valid driver's license. Salary is $57,000-$58,000.
