Job Summary
Develop and maintain relationships with individuals receiving support, families, home providers, and direct care workers. Empower individuals to express their goals and needs while facilitating successful matches between people and their care providers. Collaborate with the service planning team to create Individual Support Plans that reflect unique wishes and adhere to regulatory requirements. Oversee contracted services by completing visits in home or service environments and ensuring contractual compliance. Provide on-call support to resolve issues for providers and workers as needed. Requires a Bachelor's degree, two years of related experience, and a valid driver's license. Salary ranges from $57,500 to $58,000 annually.
