Job Summary
Develop and maintain strong relationships with individuals receiving support, their families, home providers, and direct care workers. Help individuals express their goals and needs while monitoring matches between residents and providers to ensure a supportive environment. Collaborate with the service planning team to create Individual Support Plans that reflect unique wishes and adhere to regulatory requirements. Oversee contracted services by completing visits in home or service environments and ensuring contractual compliance. Provide on-call support to providers and workers for problem resolution. Requires a bachelor's degree, two years of related experience, and a valid driver's license. Salary is $57,000–$58,000.
