Montrose: Care Coordinator

Care Coordinator to manage a client caseload and coordinate quality home care services across Western Colorado. Responsibilities include conducting intake assessments, coordinating care with clients, families, caregivers and community partners, completing client visits, supporting caregiver scheduling and coverage, maintaining documentation, and communicating with families, providers, and partner agencies to ensure compassionate and effective care. Travel up to 50% and a work-from-home setup with occasional in-office meetings and client home visits are required.
