Job Summary
Conduct regular in-home assessments to observe and record consumers' behavioral and physical signs and symptoms. Assist in maintaining adherence to physical health and psychiatric medications by enlisting natural supports and increasing motivational skills. Formulate medication management plans with consumer participation and document all activities in the electronic health record. Provide ongoing in-home instruction regarding medication benefits and side effects. Create medication lists and allergy records for wallet or purse use. Coordinate lab work and implement systems for discarding old prescriptions to enhance safety. Guide consumers in navigating care systems including Primary Care Physicians, labs, specialists, and pharmacies. Adhere to seven recognized Sanctuary commitments covering social responsibility, non-violence, and emotional intelligence.
Required Qualifications
- Licensed Practical Nurse
- Act 33/34
- FBI clearances
- Act 31 Certificate
- Valid Pennsylvania driver's license
- Acceptable Motor Report Vehicle Report
Desired Qualifications
- One year of clinical experience
- At least 2 years of professional experience working in the mental health field
