Job Summary
Mobile Medicine RN responsible for performing independent visits outside the clinic, including health assessments, medication reconciliation, wound care, immunizations, lab work, and patient education. Collaborates with mobile medicine, outreach, and primary care teams to support population health initiatives, advances health equity initiatives, and coordinates care with unengaged patients to develop care plans that improve chronic conditions and overall well-being. Requires strong communication, motivational interviewing, and ability to educate patients, families, and caregivers, while leveraging community resources to address social determinants of health. Emphasizes patient-centered, evidence-based care and alignment with CHAS Health Mission and Core Values.
Required Qualifications
- Valid Washington State RN License
- Bachelor of Science in Nursing (BSN) preferred
- Minimum of three years’ experience in clinic setting as an RN preferred
- Experience in triage and Community Health setting a plus
- Experience with electronic health records (EHR) strongly preferred
- BLS (CPR/AED) required
