Job Summary

Conduct comprehensive in-home assessments to identify medical, behavioral, and social needs, then coordinate with primary care and specialty teams to close care gaps. Provide clinical care within NJ paramedic scope, monitor chronic disease management for conditions like CHF and COPD, and support persons with SUD by bridging them to community-based health services. Identify immediate changes in patient status requiring 911 activation or transitional care intervention, and deliver post-discharge home visits to reduce hospital readmissions. Deliver education to first responders regarding resources for SUD patients, ensuring bi-lingual communication in English and Spanish as needed. Full-time, 40 hours/week schedule in Cape May Courthouse, with competitive compensation ranging from $31 to $53 per hour.

Required Qualifications

  • 3-5 years of experience required, in advanced life support (ALS) environment as a paramedic
  • Communication-Ability to communicate with patients, visitors and coworkers required
  • High School/GED required
  • Bachelor degree required, in Healthcare or Public Health related field
  • Degree requirement waived with paramedic experience of 10 years or more
  • NJ Mobile Intensive Care Paramedic (MICP) or Mobile Intensive Care Nurse (MICN) required, or eligible for NJ reciprocity
  • Community Paramedicine Certification (CP-C) required or obtain within 1 year of hire
  • BLS, ACLS, PALS, EPC, or PEPP upon hire
  • Full time, 40 hours/week
  • Cape May Courthouse

Desired Qualifications

  • Bi-lingual English/Spanish skills preferred
  • NREMT-Paramedic preferred
