Job Summary
The UVM HealthNet Community Paramedic functions as a key member of a multidisciplinary healthcare team, providing direct care to patients in various health conditions in pre and post hospital settings. Responsibilities include collaborating with local social services, case managers, and medical facilities to assist with patient treatment inside their homes and public spaces, improving patient navigation through the healthcare system to reduce readmissions and emergency service misuse. Candidates must have relevant paramedic certifications and significant patient care experience.
Required Qualifications
- High School Diploma or Equivalency (GED) required
- Nationally Registered Paramedic Certification
- 3 or more years of direct patient care experience in 911, critical care, or community paramedicine
- Vermont Office of Emergency Medical Services Paramedic Certification (required)
- National Registry Emergency Medical Technician Paramedic (NREMT-P) (required)
Desired Qualifications
- Associate’s or Bachelor’s Degree preferred
- CP-C (Community Paramedic Certification) preferred after 12-to-24 months of hire
