Job Summary

Travel extensively to community locations and outreach destinations with the mobile health team two to three days per week, providing evenings and weekend coverage as needed. Deliver primary health care services including assessment, diagnosis, treatment, prescribing, and patient education to vulnerable populations such as farmworkers, unhoused individuals, students, and seniors. Review and manage patient information, including labs, diagnostic images, and medical records, while performing various outpatient procedures and acute triage as required. Prioritize patient care throughout the day and coordinate activities with support staff. Document all interactions in the electronic health record system and respond to afterhours triage calls on a rotation basis. Collaborate with physicians, pharmacists, and care team members to optimize patient outcomes.

Required Qualifications

  • NP or PA-C with at least 3 years of experience
  • MD or DO with at least 1 year of experience
  • Graduate of an accredited Master of Science in Nursing program
  • Graduate of a Physician Assistant Program from an accredited school
  • MD/DO licensed in California with board certification
  • Current Family Nurse Practitioner certification
  • Current Physician Assistant License
  • National Commission on the Certification of Physician Assistants Certificate
  • Current California Provider certification/licensing
  • Current California medication furnishing license
  • Current DEA License
  • CPR certification
  • Strong verbal and written communication skills
  • Excellent interpersonal and customer service skills
  • Ability to treat patients with complex medical and social needs
  • Strong time management skills
  • Ability to prioritize
  • Ability to work in a fast-paced environment
  • Computer and data entry skills
  • Excellent problem-solving skills
  • Ability to work autonomously with a mobile health team away from a health center
  • Drivers License

Desired Qualifications

  • NP or PA-C with at least 3 years of experience
  • MD or DO with at least 1 year of experience
  • Experience in a Federally Qualified Health Center
  • Experience with mobile health or street medicine teams
  • Bilingual (English/Spanish) language
  • Electronic health record system experience
