Job Summary
Conduct crisis screenings, assessments, and support services for clients and families, managing a caseload that includes hospitalization deflection and post-hospitalization treatment planning. Attend 72-hour staffings and residential placement meetings while developing stabilization plans and linking clients to substitute care and community resources. Maintain continuous on-call coverage with rotating evening and weekend shifts, driving daily to hospital sites and client homes for in-home services. Provide timely clinical documentation, participate in department meetings and supervision, and collaborate with agency professionals to ensure continuity of care.
Required Qualifications
- Bachelors Degree from an accredited College or University in Social Work, Psychology, or closely related discipline
- minimum of a high school diploma or GED and 5 years supervised clinical experience in mental health or human services
- Valid driver's license
- proof of insurance

- Computer skills
- Must work a flexible schedule
- rotating direct on-call responsibilities
- minimum of 2 evening shifts
- 1 weekend shift per month
- holiday hours
Desired Qualifications
- Masters Degree from an accredited College or University in Social Work, Psychology, or closely related discipline
- One to two years experience working with children and adolescents
- Bilingual (Spanish)
