Job Summary
Conduct psychosocial assessments and develop care plans for Hamilton County residents referred by psychiatrists, case managers, physicians, police, and individuals. Provide education, consultation, and serve as a liaison to healthcare and mental health teams regarding patients' familial, emotional, psychological, social, financial, and developmental aspects. Promote community stabilization and facilitate transitions to appropriate levels of care. This part-time, second-shift weekend role supports the Mobile Crisis Team's mission to address complex needs and ensure safe discharge planning.
