Job Summary
Mobile Care LVN role providing direct on-site and remote care to Embrace members in assisted living and community settings under NP/MD supervision. Responsibilities include focused assessments (vital signs, pain, mental status, cardiopulmonary/GI), wound care, vaccination support, IV therapy within scope, catheter care, and coordination with community staff. Acts as a clinical extension of the primary care team, administering vaccinations, wound dressings, urinary catheter care, and peripheral IV management; conducts follow-ups after transitions or urgent events; maintains proactive communication with primary care and community staff, and supports care delivery workflows and member satisfaction.
Required Qualifications
- Licensed Vocational Nurse (LVN)
- Associate's Degree or equivalent experience preferred
- 3-5 years of related experience
- Ability to perform focused clinical assessments
- Vaccinations administration
- Wound care and IV therapy within scope
- Catheter care and management
- TB screening policy compliance
- Field-based, clinical team collaboration with NP/MD direction
- Strong communication and care coordination skills
