Mobile Care LVN (Maricopa, AZ)

The LVN/LPN provides direct, onsite and remote care to Embrace members residing in assisted living and community-based settings under the direction of Nurse Practitioners and Physicians. Responsibilities include clinical care and assessment (vital signs, pain and mental status, wound status, catheter function), administering vaccinations and injections, peripheral IV procedures within scope, catheter care, and escalation of changes in condition. Performs focused assessments, direct clinical interventions, monitoring for adverse reactions, and transitions of care with strong communication and coordination with the primary care team. The role emphasizes field-based nursing, a professional brand ambassador presence in assigned communities, collaborative relationships with residents, families, and staff, and adherence to applicable state and organizational regulations, all under NP/MD direction.
