Job Summary
The Minimum Data Set Licensed Practical Nurse (MDS-LVN/LPN) will conduct and coordinate the development, completion, and transmission of the resident assessment in accordance with state requirements. Responsibilities include assisting in MDS assessments, ensuring accurate submissions, collaborating with interdisciplinary teams for resident care planning, maintaining data accuracy, monitoring resident status, and ensuring MDS compliance with regulatory requirements.
Required Qualifications
- 1-3 years of MDS experience
- Nursing degree from an accredited college
- Current and unencumbered LVN/LPN license in the state
- Knowledge of clinical standards of practice, regulations, and reimbursement governing long-term care and Medicare participation
- Knowledgeable of nursing practices, procedures, terminology, laws, regulations, and guidelines that pertain to long-term care
Desired Qualifications
- Experience providing care to multiple residents
- Experience in a Skilled Nursing environment
- Experience with PDPM (Patient-Driven Payment Model)
