Job Summary
Conduct comprehensive patient and family assessments to determine hospice needs and develop individualized care plans. Administer medications and treatments per physician orders while counseling patients on self-care techniques and palliative actions. Coordinate care by communicating with physicians, community health resources, and interdisciplinary teams to ensure continuity. Fulfill assigned case management duties as an on-call RN or Case Manager, supervising ancillary personnel when required. Maintain accurate clinical documentation and actively participate in quality assessment and performance improvement teams.
Required Qualifications
- Graduate of an accredited school of nursing
- Current RN licensure in home state or multi-state
- CPR certification
- Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
- Visual acuity and hearing to perform required nursing skills
- Prolonged or considerable walking or standing
- Able to lift, position and/or transfer patients
- Able to lift supplies and equipment
- Considerable reaching, stooping bending, kneeling and/or crouching
- Excellent observation skills
- Excellent verbal and written communication skills
- Problem solving skills
- Basic math skills
- Nursing skills per competency checklist
- 100% travel Monday - Friday
- Candidate could live in any of our ten states (MN, WI, IA, SD, MO, IL, KS, NE, IN or MI)
- Assignments could be 2 weeks to 3 months with 1 week notice of location
- Must be available for on-call duties as defined by the on-call policy
- Must be able to effectively lead and coach others
Desired Qualifications
- One (1) to two (2) years of recent acute care experience in an institutional setting
- One (1) to two (2) years of recent experience in home care
- Management experience
- Responsible for supervising hospice aides
- Ideal candidates would live within one of St. Croix Hospice's ten states or a geographically connected state
- SCH will assist with licensure for other states if needed
