Job Summary
Care Navigator role for a Michigan-licensed Physical Therapy Assistant. This remote, full-time position supports patient success through remote monitoring, guiding patients in therapeutic exercise, neuromuscular re-education, and functional training via the company’s technology platform. The role requires maintaining records, providing ongoing feedback to the evaluating clinician, managing a caseload, and helping improve platform functionality. Ideal candidates are self-starters with 2+ years in musculoskeletal health, have graduated from an accredited PT program, hold an active Michigan PTA license (and a PT license in a compact state), and are comfortable working across multiple systems (EHR, scheduling, messaging). Strong communication, relationship-building, and data-analytic abilities are essential, as is the ability to work independently in a fast-paced startup environment and a genuine passion for patient user experience.
Required Qualifications
- Graduated from an accredited physical therapy program
- Active PTA license in Michigan
- Active PT license in a compact state
- Tech Savvy in multi-platform environments (EHR, messaging, scheduling)
- Experience in behavioral modification is a plus
- Excellent communication and interpersonal skills; ability to present meaningful data to drive decisions
- Ability to multi-task and manage diverse user groups
- Independent work style with limited supervision
- Willingness to work in a fast-paced, change-driven environment
