MHSU Care Manager (Mobile Position - Gastonia, NC)

Job Summary

Provide education, referrals, and care management for adults and children regarding physical health, behavioral health, and support services. Conduct comprehensive assessments, create care plans, and facilitate connections to primary healthcare through community resources. Coordinate care team meetings, monitor progress through in-person and collateral contacts, and oversee transitions between clinical settings. Identify service gaps, refer members to community resources, and maintain accurate tracking of care outcomes and waiting lists. Collaborate with stakeholders including DSS, school systems, and other community partners to address system barriers and facilitate high-risk team meetings.

Required Qualifications

  • Bachelor's degree in a human service field
  • at least two years of full-time experience with the population served
  • Bachelor's degree in a field other than human services
  • at least four years of full-time experience with the population served
  • Master's degree in a human service field
  • one year of full-time experience with the population served
  • Current unrestricted LCSW-A, LCMHC-A, LCAS-A, LMFT-A
  • Current unrestricted LCSW, LCMHC, LPA, LMFT, LCAS, or RN licensure
  • appropriate professional board of licensure in the state of North Carolina
  • Must reside in North Carolina
  • Must have ability to travel as needed to perform the job duties

Desired Qualifications

  • Extensive understanding of the Diagnostic and Statistical Manual of Mental Disorders (current version)
  • Considerable knowledge of the MHSU/IDD service array provided through the network of the LME/MCO's providers
  • Knowledge of LME/MCO's implementation of the 1915(b/c) waivers and accreditation
  • Excellent computer skills including proficiency in Microsoft Office products (Word, Excel, Outlook, and PowerPoint)
  • Exceptional interpersonal and communication skills
  • Excellent problem solving, negotiation, arbitration, and conflict resolution skills
  • Detail-oriented, able to organize multiple tasks and priorities and effectively manage projects from start to finish
  • Ability to make prompt independent decisions based upon relevant facts, to establish rapport and maintain effective working relationships
  • Ability to change the focus of his/her activities to meet changing priorities
  • A high level of diplomacy and discretion is required to effectively negotiate and resolve issues with minimal assistance
