Job Summary
Support consumers in achieving personal care goals, educate them on medications, and track logs while conducting monthly audits. Coordinate services based on individual needs and deliver high-quality care aligned with Individual Rehabilitation Plans. Transport consumers using the company van as required and accurately document all services within timeframes using electronic health records. This role involves a competitive hourly rate of $18.00–$18.75 and requires a valid driver's license with post-offer verification. Candidates must hold a Bachelor's degree in a related field or equivalent experience.
Required Qualifications
- Bachelor's Degree in mental health, psychology, sociology, or social work, human services or a related field from an accredited college or university
- A combination of one (1) or more years' of college, plus related life or work experience equal to four (4) years
- High school diploma or GED plus four (4) years of related work or life experience
- Strong verbal and written communication skills
- Proficiency in Microsoft Office Suite, including Outlook, Excel, and Word
- Valid driver's license
- Driving eligibility consistent with company insurance requirements
