Job Summary
Outreach Case Manager responsible for daily street outreach, engagement with clients to assess housing, health, and supportive service needs, and linking them to long-term housing, health, and other services. Conduct VI-SPDAT assessments, develop individualized service plans (ISP), provide housing navigation and advocacy with landlords, assist with documentation and benefits (GR, SSI, TANF, Veterans Pension), and coordinate with mental health and substance-use providers. Build relationships with community partners, attend meetings, and document client progress in HMIS and other program systems. Requires driving, collaboration with a multidisciplinary team, and delivering high-quality case management and customer service. Based on Union Station area; standard full-time schedule with standard business hours; on-site work with some mobility to serve clients.
Required Qualifications
- Two (2) years of experience in outreach case management or a related field
- Have employment eligibility verification
- CPR/First Aid training or ability to obtain
- Tuberculosis Test
- Background Screening
- Drug Test
- A valid driver’s license
- Proof of insurance and ownership for personal vehicles used during work duties
- Driving is an essential function of this position
- Ability to meet PATH's insurance requirements
- Authorized to work for any employer in the U.S. (no sponsorship)
