Job Summary
Escort clients to groups and activities, maintaining a 1:6 ratio while recording participation on a tracking calendar. Facilitate assigned groups, lead MHW discussions, and conduct daily community meetings. Reinforce appropriate behaviors and coping skills, support residents in overcoming personal barriers, and assist with completing forms. Document interventions and interactions in medical records, assign points, and complete weekly evaluations identifying short-term goals. Report behavioral changes to the charge nurse and Program Counselor. Collaborate with in-unit staff to ensure consistent care. This role requires at least six months of experience in a similar health facility setting and the ability to make independent decisions tactfully. Generations Healthcare provides training for motivated candidates in group facilitation and caseload management.
Required Qualifications
- at least 6 months job experience in a similar health facility (SNF, ALF or Hospital setting)
- ability to read, write, speak, and understand the English language
- ability to make independent decisions when circumstances warrant such action
- ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public
