Job Summary
Assess and develop treatment plans for individuals served, including after-care planning and closure. Provide after-hours crisis response as needed and utilize activity-based interventions to support families facing out-of-home placements or reunification. Build relationships with referral agencies, guide families in accessing community resources, and participate in team meetings. Manage documentation for accuracy and submission while potentially providing training, field supervision, and reviewing peer records. Maintain a caseload of 10 to 12 individuals with productivity requirements of 15 to 17 hours weekly.
Required Qualifications
- Master's Degree in Human Services, Social Work, Psychology or related field
- BBS Registration - AMFT, ACSW, APCC
- Familiar with electronic health record and competent in basic computer skills
- Current driver's license, car registration, and auto insurance
- Successful clearance of local and/or state background checks
