Job Summary
ACT Therapist on the Montgomery County ACT Team providing clinical, community-based mental health services to people experiencing homelessness or at risk, including assessments, individual/family/group therapy, community outreach, home visits, and coordination of admissions and assessments. Develop and revise treatment, crisis, and discharge plans; maintain timely clinical documentation; support daily living skills and housing stability; collaborate with a multi-disciplinary team to promote recovery, stability, and community reintegration. Requires Maryland licensure (LMSW/LGPC/LCSW-C or LCPC), strong organizational and communication skills, and willingness to work in a community-based setting; Driver's License is required. Bilingual English/Spanish strongly preferred.
Required Qualifications
- Maryland licensed LMSW, LGPC, LCSW-C or LCPC
- Experience in case management or counseling of people with psychiatric disabilities, histories of substance abuse and/or trauma
- Driver’s License required
