Job Summary
Provide psychosocial assessment, individual/family therapy, discharge and aftercare planning to patients and families in an outpatient setting. Deliver services based on recovery-oriented models, family systems, and evidence-based treatments while serving as the primary contact for patient and family education. Act as a liaison to community programs to develop strategic partnerships that enhance aftercare plans. Conduct consultation and training on psychosocial aspects of human behavior and complex environments. Make timely referrals for specialized evaluations and programs. Work independently within a team environment, prioritizing assignments and ensuring patient privacy. Requires a Master's degree in social work, counseling, or related fields, plus LMFT, LPCC, or LCSW licensure and two years of recent relevant experience.
Required Qualifications
- Master's: social work, counseling, or related field
- LMFT-Licensed Marriage & Family Therapist OR LPCC-Licensed Professional Clinical Counselor OR LCSW-Licensed Clinical Social Worker
- 2 years of recent relevant experience
- Knowledge of various principles, procedures, views, and equipment used in the treatment of patients with varying disorders
- Ability to interpret a variety of data and instructions, furnished in written, oral, diagram, or schedule form
- Demonstrates working knowledge of available specialized treatment centers including mental health and substance abuse services and facilities
- Possess written and verbal communications skills to explain sensitive information clearly and professionally to diverse audiences, including non-medical people
- Well-developed time management and organizational skills, including the ability to prioritize assignments and work within standardized operating procedures and scientific methods to achieve objectives and meet deadline
- General knowledge of computer applications, such as Microsoft Office Suite (Word, Excel and Outlook), electronic health records (EHR)
- Ensure the privacy of each patient's protected health information (PHI)
- Build collaborates relationships with peers and other healthcare providers to achieve departmental and corporate objectives
- Make timely referrals for specialized evaluations, programs, and services
