Job Summary
Conduct comprehensive psychological and behavioral case management services within a continuum of high to low intensity care designed to improve or maintain consumers' abilities to function in the community. Assess, monitor, and follow up with consumers on their progress toward recovery while managing all clinical documentation. Provide psychosocial support including goal setting, daily living skills instruction, and assistance with accessing basic needs such as food, shelter, and medical care. Arrange transportation to community resources and coordinate with families, employers, and support systems to maximize treatment engagement. Reach out to eligible consumers to educate them on available services and maintain a trauma-informed environment. Work 8am–5pm Monday through Friday in a hybrid schedule at the Denver location.
Required Qualifications
- Bachelor (MHTI) or Master (MHTII) Degree in psychology, social work, or other related human services degree
- Must have a valid Colorado Driver's license
- Recent clean driving record
- Ability to transport the people we serve in your vehicle
- Ability to obtain and carry Business Use coverage on your auto policy
