Job Summary
Conduct initial patient visits of 50 minutes or less, evaluate functioning levels, determine risk factors including suicide/homicide potential, and provide crisis intervention. Deliver evidence-based assessments, diagnoses, and interventions for mental health and substance use conditions using techniques such as motivational interviewing and behavioral activation. Manage a caseload of patients requiring ongoing support while facilitating access to new patients and coordinating referrals to external specialty providers. Document services in electronic records and track caseloads to identify patients needing outreach. Collaborate with primary care teams and psychiatric consultants to support integrated care models.
Required Qualifications
- Master's degree in Social Work, Counseling, or related field
- one to three years related experience working with underserved populations and/or high-risk groups
- significant training in integrated care through Master's level program or prior experience working in primary care providing mental health services
- current credential as a Licensed Social Worker or Licensed Social Worker Associate
- Licensed Mental Health Counselor or Licensed Mental Health Counselor Associate
- Licensed Marriage and Family Therapist or Licensed Marriage and Family Therapist Associate
- full licensure (Licensed social worker, licensed mental health counselor, and Licensed Marriage and Family Therapist) will be obtained within 6 months of hire
- Proof of vaccination for COVID-19
- proof of immunizations and/or immunity to MMR (measles, mumps, rubella)
- proof of immunizations and/or immunity to Varicella
- annual Influenza
- TB QuantiFERON Gold Titer
- Tdap (within last 10 years)
- Hepatitis B

- Demonstrated ability to collaborate effectively in a team setting
- Comfort with the fast pace of primary care
- Ability to effectively communicate with primary medical care providers about mental health issues treatment
- Ability to effectively engage patients in a therapeutic relationship
- Experience with screening and assessment for common mental/substance use disorders
- Working knowledge of differential diagnosis of common mental/substance use disorders
- Working knowledge of evidence-based psychosocial treatments for common mental disorders
- Working knowledge and experience with brief, structured counseling techniques (e.g. Motivational Interviewing, Behavioral Activation)
- Basic knowledge of psychopharmacology for common mental disorders
- Knowledge of psychological assessment, DSM V, and chemical dependency
