Job Summary
Conduct comprehensive biopsychosocial/spiritual assessments, diagnoses, and treatment interventions for patients and support systems. Collaborate with the Inter/Multi-Disciplinary Team to develop and implement evidence-informed person-centered care plans across the care continuum. Work with inpatient and outpatient populations while respecting patient self-determination and reducing health disparities. Adhere to the NASW code of ethics and Standards of Practice. Operate within the Providence Home and Community Care service line, which provides longitudinal care and end-of-life support across seven states.
Required Qualifications
- Master's Degree in Social Work from a CSWE Accredited School of Social Work, OR from a school in candidacy status by the council of Social Work Education (CSWE)
- Oregon Clinical Social Worker License
- Washington Clinical Independent Social Worker License
- License in states wherever care is delivered
- 2 years relevant Social Work experience
