Job Summary
Provide comprehensive psychosocial rehabilitation services to individuals with severe and persistent mental illnesses, focusing on increasing functional skills, reducing symptoms, and promoting independence. Deliver day-to-day physical, emotional, and social supports including personal care, community appointments, and meal planning. Collaborate with an interdisciplinary team to develop individualized treatment plans, monitor client progress, and maintain a safe environment. Establish therapeutic relationships based on respect while advocating for resource access and minimizing challenging behaviors. Contribute to team meetings, quality improvement activities, and client evaluations within a client and family-centred recovery model.
Required Qualifications
- Grade 12
- Social Services Diploma
- Certificate from a recognized Health Care Worker educational program, such as Health Care Support Worker Certificate, Certificate in Community Mental Health, Human Service Worker Certificate or Addictions Worker Certificate
- two (2) years' experience working in a psychiatric setting with individuals who have complex mental health and addictions issues
- other qualifications determined to be reasonable and relevant to the level of work
- Ability to communicate effectively both verbally and in writing
- Ability to deal with others effectively
- Physical ability to carry out the duties of the position
- Ability to organize the work
- Ability to operate related equipment
- Successful completion of a Food Safe certification course if applicable to the designated program during their orientation/qualifying period
- Successful completion of a Criminal Records Review Check
- Report of past receipt of certain vaccines or history of certain infections
