Job Summary
Mental Health Rehabilitation Specialist provides intensive field-based rehabilitative and supportive services to adults experiencing serious mental illness, homelessness, and co-occurring conditions enrolled in a Full Service Partnership program. Working as part of a multidisciplinary team, the Rehabilitation Specialist supports clients in achieving stability, independent living, and recovery through a “whatever it takes” approach consistent with DMH and Medi-Cal requirements. Services are delivered primarily in the field and include mental health rehabilitation, targeted case management, and crisis intervention. Responsibilities include conducting field-based work, client screenings, developing and monitoring care plans with a multidisciplinary team, delivering rehabilitation and skill-building services, group rehabilitation, ongoing case management and linkage to internal/external resources, accompanying clients to appointments, assisting with benefits and housing, providing crisis intervention, maintaining documentation and reports, participating in meetings, coordinating outreach, and ensuring safety and regulatory compliance.
Required Qualifications
- Bachelor’s degree
- four (4) years of experience in a mental health setting
- two (2) years of graduate education may be substituted for the required experience OR two (2) years of post–Associate degree clinical experience may be substituted for the educational requirement
- Two years of experience working in the Skid Row community or similar high-need environment
- Ability to work in high-acuity, field-based environments
- Knowledge of trauma-informed care, harm reduction, and Housing First principles
- Strong organizational, time management and documentation skills
- Cultural humility and ability to work with diverse populations
- Experience with Electronic Health Records (EHR) systems
- Valid California driver’s license
- Experience working in a DMH funded program
- Bilingual (English/Spanish) ability preferred
- Vaccination against COVID-19 recommended by employer
