Job Summary
Mental Health Professional – BA coordinates with a multidisciplinary team to deliver recovery-oriented services in a community mental health setting. Responsibilities include case management, treatment planning, resource assistance, crisis intervention and stabilization, psycho-social assessments, risk assessments, patient and family psycho-education, development of treatment plans with measurable outcomes, documentation in accordance with contractual requirements, and providing in-home and community-based support. Requires regular driving, participation in a 24/7 on-call rotation, and collaboration with behavioral health resources to connect clients to inpatient facilities as needed. The role supports clients with coping strategies, advocacy, and person-centered counseling across a caseload that may involve individuals with schizophrenia spectrum disorders, substance use challenges, trauma, and related complexities. The position is full-time (40 hours/week) with day/evening shift options and scheduled rotations, and falls under the PACT (Program for Assertive Community Treatment) model.
Required Qualifications
- Bachelor’s degree in mental health related field
- Five (5) years of experience working under a licensed Mental Health Professional
- Driver License/Abstract
