Job Summary
Provide compassionate, whole-person care through individual and family counseling to help patients manage stress, improve relationships, and develop healthier coping strategies. Seamlessly connect clients with long-term therapy, psychiatric services, or vital outside resources to ensure continuity of care. Collaborate closely with medical providers to gain a holistic understanding of each patient's health journey while championing emotional wellness through evidence-based, strengths-focused approaches. Raise awareness of mental health services via client-driven wellness initiatives, proactive check-ins, workshops, and educational programs. Identify and address social determinants of health by bridging the gap to necessary community resources to enhance overall well-being. Cultivate strong, collaborative relationships with health center staff and leadership while adapting to evolving center and patient needs.
Required Qualifications
- A Master's in Social Work (M.S.W.) from a CSWE-accredited program—or a related behavioral health field
- A current, active license with the Board of Social Work (or related licensing board) in good standing within practice state
- At least 12 months of recent (within the last 2 years) experience providing evidence-based individual counseling in a mental health or healthcare setting
- Comfortable navigating electronic medical records, clinical documentation, and basic word processing/data entry
Desired Qualifications
- Previous experience providing integrated behavioral health services in a primary care setting within the last 3 years
